Beyond 'gum disease'

Periodontitis involves loss of periodontal attachment and supporting bone. Modern clinical assessment considers severity, complexity, distribution and evidence relevant to progression risk.

Probing and attachment

Periodontal probing helps assess sulcus or pocket depth and bleeding. Clinical attachment level incorporates the relationship between the gingival margin and the attachment apparatus.

Radiographs

Radiographic bone levels can contribute to assessment but do not replace clinical examination. Images are selected when justified by the diagnostic question.

Long-term thinking

Treatment is not complete when inflammation improves. Periodontitis generally requires a long-term maintenance strategy and continuing risk control.

Clinical assessment

Good diagnosis combines the patient’s history with direct examination and, when justified, appropriate tests or imaging. A single symptom, photograph, product claim or search result should not be treated as a complete diagnosis. Disease activity, anatomy, previous treatment, medical context and the patient’s goals can change the interpretation.

Prevention before repair

Where disease can be prevented, arrested or risk-reduced, the preventive pathway deserves explicit attention. That can include plaque disruption, appropriate interdental cleaning, fluoride exposure where indicated, diet-frequency changes, salivary management, tobacco cessation support and professionally determined recall or maintenance.

Complexity does not justify rushing the patient. Explain the finding in understandable language, distinguish what is known from what is uncertain, discuss reasonable options, obtain consent and provide appropriate pain and anxiety control.
